​​​​​​​Our staff will not come to your home if we are experiencing any signs of illness. Please let us know if anyone at your home has been sick with fever or respiratory signs, so that we may take appropriate precautions.

We no longer require masks to be worn at appointments as long as everyone has been healthy. Often our doctors choose to wear masks for their own comfort. If you would prefer your doctor wears a mask, she will be happy to do so.

To help our doctors work efficiently, we ask that our consent form is completed ahead of time. This can be submitted electronically or printed to have a hard copy to give the doctor at your appointment. You can find the consent form by clicking HERE.
